  • Zoom recording without a transcript. Box folder 2026-09-08 15.29.06 Zoom Meeting with Derek Eichenwald _ Triumph Capital_ timothy gelling (id 416326420268) contains audio, video, chat and a recording config, but no .transcript.vtt. Read.ai was in the meeting — the chat log shows "Derek and Tim added Meeting Analytics from Read to the meeting" — so a Read.ai report email may arrive after this window. Not logged; audio is never transcribed here. This is Derek's third recorded Tim Gelling call since 2026-08-11, and the open item "whether JT takes the Tim Gelling conversation" is still waiting on Derek.
  • Duplicate extension 817. Chad's firm-wide extension table assigns 817 to both Daniel Mayer and Stephen Murphy; Murphy's row is highlighted yellow in the original, which reads as Chad's own flag. Someone has to move.
  • Form 5558 ambiguity. Nydia enclosed a Form 5558 with their resignation letter. The email does not say whether Nydia actually filed the extension or merely handed over a blank for the Firm to file. The 2025 Form 5500 date of 2026-10-15 depends on the answer.
  • Replacement TPA still unowned. Six days after the 2026-09-03 heads-up and now with a formal resignation letter in hand, no successor firm and no owner has been named.
  • Two unretrievable items in the Brandon chat. The image he sent for the deck's bond section, and whatever JT sent back with "Here you go." Neither is recoverable through the connector, so the record notes the exchange without its content.

Errors

  • Microsoft Graph returned HTTP 429 three times during the run: once on the second Teams page, once on a Read.ai sender search, and once on the first MBTC calendar lookup. The Teams scan covered 36 of 50 chats on the retry before stopping; the calendar lookup succeeded on retry. The Read.ai search was not retried — the full inbox listing had already been paged in its entirety and contained no message from executiveassistant@e.read.ai, so no report existed in the window. Same rate-limit pattern flagged in the 2026-09-08 digest.
